How Formcrest meets its obligations under the EU Digital Services Act, including how to report illegal content and appeal our decisions.

Scope

Formcrest is a hosting service that lets customers collect submissions from their own forms, so we fall within the scope of the EU Digital Services Act (DSA). We comply with the DSA's obligations for hosting services. We are well below the threshold for a Very Large Online Platform (VLOP), so the additional obligations that apply only to VLOPs do not apply to us. This page sets out the contact points and procedures the DSA requires.

Single point of contact (Article 11)

Authorities, recipients of our service, and other interested parties can reach our designated point of contact for DSA matters by email at info@formcrest.com. Communications may be made in English.

Notice and action: reporting illegal content (Article 16)

If you believe a form or content hosted on Formcrest is illegal or violates our Acceptable Use Policy, you can notify us. Our report-abuse form is our designated notice-and-action mechanism under Article 16, or you can email info@formcrest.com.

To help us act quickly, please include enough information to locate and assess the content: the form or URL involved, an explanation of why you consider it illegal or in breach of our policies, and a way to contact you. We review reports promptly and take action where it is warranted.

Statement of reasons (Article 17)

When we remove content or suspend a user for violating the law or our Acceptable Use Policy, we provide the affected user with a written statement of reasons. The statement explains what action we took, the grounds for it, and a link to the path for appealing the decision.

Internal complaint-handling and appeals (Article 20)

If your account has been suspended or content has been removed and you believe the decision was wrong, you can appeal through our internal complaint-handling system by emailing info@formcrest.com. You may submit an appeal within six months of the decision. We review appeals fairly and respond within five business days. If we find the decision was mistaken, we reverse it without delay.

Transparency reporting (Article 15)

We publish information about the content moderation we carry out, including reports we receive and actions we take, on our transparency report page.
